Jumper Wire Former

We've got a couple of old jumper wire formers here at our workplace that I'm looking for a part for. They're originally from the Eraser Co. They no longer have any parts for this. I'm going to attach 2 photos that show the former and the part we need. If anyone has any information, I would appreciate it.
